Here you can find a comprehensive list of all application, designation, and other important forms, along with select informational handouts.

Please note that many forms can only be obtained by coming to the Honors Program Office.

Apply to the Honors Program

• Current Student Application - Current students can apply to the Honors Program through My.IllinoisState.edu

Academic Forms

• Plan of Study Template - Use this form to help plan your course of study at Illinois State in partnership with your advisor. This is a Word document.
• Honors Program Requirements - Informational handout summarizing the academic requirements of all Honors students.

Designation Forms

• Graduation Designation Form - This form must be returned in order to be recognized for Honors Program Scholar with Merit, Honors Program Scholar with Distinction, and Departmental Honors.
• Sophomore Honors Scholar Designation Form - This form must be returned in order to be recognized for the Sophomore Honors Scholar designation.
• Honors Designation Summary - Informational handout summarizing various designations and their requirements.=

Involvement

• Service Learning Application - This form must be returned to participate in service learning courses (Presidential Scholars do not need to return this form)

Presidential Scholars

• Faculty Colleague Plan of Study cover sheet- This cover sheet is used with the required four or five year Presidential Scholar plan of study.

• Scholarly Travel Program - Complete this form to apply for support of travel to present research or engage in scholarship.
• Letter of Recommendation Release Form - From the the Office of the University Registrar.
